Number of Awards & Eligibility: Two awards offered annually, one to a male student and one to a female student.
In order to qualify, students must meet the following criteria:
  1. This award is for U.S. students.
  2. Must be a U.S. citizen or permanent resident.
  3. Must be a high school senior.
  4. Must have cystic fibrosis.
  5. Must be an athlete and have athletic ability in the area of running. The student-athlete should jog on a regular basis and be training for the 1.5 mile qualifying run. All candidates will be judged on the basis of time.
  6. Must demonstrate service to the community.

Description: This scholarship is open to U.S. high school seniors who are athletes and have cystic fibrosis. Applicants must demonstrate scholastic ability, athletic ability (as evidenced by regular exercise), character, leadership, service to the community, need for financial assistance, and daily compliance to cystic fibrosis therapy.

Additional Information: Applicants must demonstrate scholastic ability, athletic ability (as evidenced by regular exercise), character, leadership, service to the community, need for financial assistance, and daily compliance to cystic fibrosis therapy.

Award Amount: $10,000 annually, non-renewable.  The total dollars awarded for this scholarship is $10,000.

Applications are available on the Boomer Esiason Foundation website. As part of the online application, the student must provide the following materials: a completed 'Exercise for Life' training log (available within the application); an essay of no more than 300 words discussing the importance of compliance to cystic fibrosis therapies and what he/she practices daily to stay healthy; an essay of no more than 300 words discussing his/her post-graduation goals; a recent photograph; a letter from his/her doctor confirming diagnosis of cystic fibrosis; a transcript (high school, college, and/or graduate school); a letter of acceptance from his/her institution; a detailed breakdown of tuition costs from academic institution; and W2 forms for both parents (as appropriate) or a tax return if self-employed. Applications must be returned to the address provided by 5:00 p.m. on the deadline date.
