In order to qualify, students must meet the following criteria:
  1. This award is for U.S. students.
  2. Must be a resident of Rhode Island.
  3. Must be diagnosed with epilepsy.
  4. Must be a U.S. citizen or permanent resident.
  5. Must be a high school senior at time of application.
  6. Must attend high school in Rhode Island.

Additional Information: The Matthew Siravo Memorial Foundation, Inc. is a non-profit organization established in honor of Matthew Siravo, a five-year-old boy, who had epilepsy. The foundation's mission is to raise the awareness of epilepsy, award annual scholarships to high school seniors in Rhode Island with epilepsy and provide funding for continued epilepsy research.

Award Amount: $2,500 annually, non-renewable  The total dollars awarded for this scholarship is $10,000.

Applications are available from the Matthew Siravo Memorial Foundation website. In addition to the completed application, each student must submit the following: an essay of no more than 250 words in length (typed and double-spaced) on one of the topics described on the application or a topic of his/her own choice; a letter from his/her doctor indicating that he/she does in fact have epilepsy; two letters of recommendation from an individual other than his/her doctor; an unofficial copy of his/her high school transcripts; and an acceptance letter from his/her intended college, which may be submitted either with the packet or at a later date. The applicant should also include a small photograph of himself/herself and a letter stating permission to use the scholarship award winner's name and photo on the Matthew Siravo Memorial Foundation website and other media outlets. The letter must be signed by the applicant if he/she is 18 years old, or by his/her parent/legal guardian.
