Number of Awards & Eligibility: Two awards offered annually. One award will be provided to a male applicant, and one will be provided to a female applicant.
In order to qualify, students must meet the following criteria:
  1. This award is for U.S. students.
  2. Must be a high school senior at the time of application.
  3. Must attend Poughkeepsie High School in Poughkeepsie, New York.
  4. Must demonstrate academic achievement.
  5. Must be a U.S. citizen or legal permanent resident.

In order to qualify, students must meet the following criteria:

Description: This award is available for current high school seniors graduating from Poughkeepsie High School in Poughkeepsie, New York. Applicants must demonstrate academic achievement and a history of participation in sports or other community activities.

Additional Information: Established by John Csizmar, scholarships will be awarded to one graduating male senior and one graduating female senior from Poughkeepsie High School who have achieved a strong academic record, and have participated in high school sports or and/or community service projects.

Award Amount: $3,500 annually, non-renewable.  The total dollars awarded for this scholarship is $3,500.

Applications are available online from the Community Foundation of Hudson Valley website at the address provided.In addition to the completed application, each applicant must submit two letters of recommendation (one academic and one personal); a copy of his/her most recent transcript; a copy of the student's Student Aid Report (SAR) and Expected Family Contribution (EFC) information; and an essay of approximately 500 words responding to the prompt listed on the application.The Student Aid Report (SAR) is obtained by filing the FAFSA (Free Application for Federal Student Aid). The FAFSA is available online at www.fafsa.ed.gov, at high school guidance offices, and at college financial aid offices.Applications must be completed online by the deadline date.
